What Causes Hearing Loss? Conductive vs Sensorineural
Hearing loss is not just one thing. It comes in different types. Understanding the cause helps us figure out the right solution. A skilled best audiologist can tell the difference with a simple test.
Conductive hearing loss happens when sound cannot travel freely through your outer or middle ear. Something is blocking the path. The inner ear works fine. The nerve works fine. But the sound never reaches them properly.
One of the most Common causes of conductive loss is earwax buildup. This is very common in Hyderabad's climate. Too much wax can completely block the ear canal. A simple cleaning by an audiologist often restores hearing immediately. Fluid in the middle ear is another cause, especially after a cold or infection. The fluid stops the eardrum from vibrating properly. A hole in the eardrum from an old injury or infection can also cause conductive loss. So can problems with the three tiny bones in the middle ear. These bones can harden or break apart over time.
The good news about conductive loss is that it is often treatable. Medication, a small procedure, or even just cleaning the ear can fix it. No hearing aid needed.
Sensorineural hearing loss is different. This happens when the inner ear or the hearing nerve itself is damaged. The most common type by far. Age is the biggest factor. As we get older, the tiny hair cells in our inner ear wear out. They do not grow back. Noise exposure is another big cause. Years of loud music, heavy traffic, or factory work can damage these hair cells permanently. Certain medications, illnesses like meningitis, and head injuries can also cause sensorineural loss.
Unlike conductive loss, sensorineural loss is usually permanent. But it can be managed very well with modern hearing aids. A best audiologist can program these devices to match your exact pattern of loss.
Some people have mixed hearing loss. That means both conductive and sensorineural issues at the same time. Part of the problem is in the outer or middle ear. Part is in the inner ear. Your audiologist will explain which type you have and what can be done about it.
Free Hearing Test: PTA and Impedance Testing
You do not need a prescription or a referral. Just walk into our Habsiguda clinic. We offer a free hearing test that takes about 30 to 40 minutes. No charge for the evaluation itself.
The main test is Pure Tone Audiometry or PTA. You sit in a quiet room. You wear headphones. You listen for beeps and tones at different volumes and pitches. Whenever you hear something, you press a button or raise your hand. Simple. The audiologist controls the test from the other side, making sounds softer and softer until you stop responding.
PTA tells us your hearing levels across different frequencies. It creates a map of your hearing. But it does not tell us where the problem is located. That is where impedance testing comes in.
Impedance testing, also called tympanometry, checks how well your eardrum moves. A small probe sits at the opening of your ear canal. It changes air pressure slightly and measures how your eardrum responds. This test tells us if there is fluid behind the eardrum, a hole in the eardrum, or problems with the middle ear bones. It is quick and painless. You barely feel a thing.
Here is something important to know. The hearing test itself is free. But if you need a printed copy of your audiometry report for your records or to show another doctor, there is a small charge for that. We explain this upfront. No surprises.
How to Read an Audiometry Chart
After your test, our best audiologist in Habsiguda will show you a chart called an audiogram. It looks like a grid. Let us explain how to read it so you understand your own results.
The horizontal axis, running left to right, shows frequency or pitch. Low frequencies on the left, around 125 or 250 Hz. High frequencies on the right, going up to 8000 Hz. The vertical axis, running top to bottom, shows loudness in decibels. Soft sounds at the top, around 0 to 20 dB. Very loud sounds at the bottom, around 90 to 120 dB.
Your results are marked with symbols. Circles or O usually represent your right ear. Crosses or X represent your left ear. These symbols are placed on the grid based on the softest sound you could hear at each frequency. The symbols are then connected with a line. That line is your hearing pattern.
If your line stays near the top of the grid, between 0 and 25 dB, your hearing is within normal range. If the line drops to 40 dB, you have mild loss. You miss soft sounds and some speech. If the line drops to 70 dB, you have moderate to severe loss. You need loud sounds to hear anything. If the line drops below 90 dB, that is severe to profound loss.
Now look at the shape of the line. Does it slope downward on the right side? That means you have high frequency loss. Common with aging and noise exposure. You hear low pitched sounds but struggle with high pitched ones. Does the line look flat across all frequencies? That means your loss affects all pitches equally. Does the line dip only in the middle? That is a mid frequency loss, less common but it happens.
Your audiologist will also show you bone conduction results on the same chart. These are marked with brackets like < and >. If your air conduction line is low but your bone conduction line is normal, you have conductive loss. The problem is in your outer or middle ear. If both lines are low and look similar, you have sensorineural loss. The problem is in your inner ear or nerve.
This chart is your hearing fingerprint. No two people have exactly the same pattern. That is why modern hearing aids are programmed individually for each person.
